name change
I wish to change my name back to my maiden name. Is there a simple form I can fill out to do so? If so, where can I get a copy of the form? Then, what procedure do i follow?

Suggest you see the attorney who represented you in your divorce. He may be able to accomplish your wish at minimal expense. Otherwie see the Clerk of the Circuit Court nearest you.
