Name Change
I am doing a general name change because I want to change my first name, is there a form that I can fill out or is this a longed out procedure. I thought this can be done without a lawyer however where can I find the form to do this.

I would suggest that you visit the closest law library. When an attorney does not know what information a pleading must contain, that is usually where he or she goes to try and find information or sample forms.
