Name change for minor

While separated from my husband I had a

child with someone else. My son now has

the last name of the other man who has no

legal ties (his name doesn't even appear in

the birth certificate, he has no contact with

us, and I don't even know where he is at),

what do I need to do to change my son's

last name?
