Legal Question in Family Law in Florida

name change

my mother married when I was 10 and changed my name, without any legal documents, to her new married name. I don't have any document that connects me to my birth certificate. Can a simple name change form take care of this problem. Currently live in Florida - actions took place in NY.

If your birth name was never legally changed in a court of law, you can simply resume using it. I don't know what you mean by "I don't have any document that connects me to my birth certificate." If you do not have your birth certificate, you can get copies by contacting the bureau of vital statistics where you were born.
