How can i change my last name to my mother's last name?

I'm a 15 yr old sophomore with very high goals set for myself. My goals are to attend Harvard or Yale medical school. Everything i do is to help me accomplish these goals i've traveled with people to people ambassador programs and this upcoming summer I'm attending Harvard's summer program. My father gave up all parental responsibilities when i was 13. And i dont want my high school or college achievments to be in his name. How can i go about changing my last name?
