Name Change Question

I was never given a middle name at birth, but now would like to have one. What is the legal process to add a middle name? Do I have to hire a lawyer and see a judge to do this or can I just fill out some forms? Thank you.

You can do it without an attorney, but you do need to file a petition in court.
