changing my 12 year olds last name to mine

my son is 12 and we have had no contact with his biological father in 11 years. no idea where he is. we have been divorced since 1997. i remarried and now my son would like for his name to be changed to ours. how do i go about doing this?

Simply petition for a name change. You have to notify the other parent but it can be done by publication if you don't know where he is.
