Child Passport/Custody

I want to take my child out of the country on vacation, I want to get a passport, I do not know where her father is, he has paid child support for 4 months through an income deduction order (they must have found him after 5 years) but has had no contact in four years. I have primary and sole residential custody, how can I get a passport when I need a notarized letter from him and I have no clue where he is, what are my options, if any? Also since he has had no contact in 4 years what are my odds of getting his rights completely revoked, I understand I will give up child support I do not care.

If your present court order is insufficient, you will need to apply to the court for sole custody.

To answer your passport question, The Passport Bureau has a form you can fill out in case the other parent is not available to give consent. I think it's DS-3053 and you can print it right from their website when you are ready to deal with your application.
