childbirth/death of the mother

I am a pregnant single mother of triplets. What legal form must I fill out in case something should HAPPEN TO ME during labor or delivery, causing my death.. and should the babies live. Id like to know and see a form that I need to fill out turning over custudy of the babies to their biological father, who may or may not be there at delivery. I want him and his family to have responsibliy of the babies should they live and something happen to me during the procedure. Im 48 and considered high risk.. because of my AGE ONLY.. and the fact there are triplets.

Since they are not born. a guardianship in utero won't work. Best thing is for him to establish paternity as soon as it is feasible. Then all his rights to the triplets kick in.

Also a "special" power of attorney" might work forseeing that the hospital may be a bit anxious as to whom they would turn over the chx. Of course, your parents might stand in.
