Your child is 4 years old and probably doesn't have the wherewithall to make accurate statements on that issue. I suggest you hire someone to watch your child's mother and if you can provide some evidence of improper parenting on her part, then you could file for change of custody and supervised custody for her. Also, if your agreement filed with the court states no exposure to paramours - then you could very well file a contempt/enforcement against her.

You will need evidence outside of the child's statements. If you file a Petition the Court it is not likely a child of such a young age will be allowed to testify in any form. You may want to make your own observations and ask questions of your ex as to where your child is sleeping whether she has her own bedroom etc. Once you have the evidence you need you may need to file a Petition to Modify the Time Sharing plan and ask the court to put restrictions/guidelines on how your ex time shares with the child when she is in her custody.
