Custody of child
I need to know what papers to file or how to go about getting custody of my daughter and if possible how to terminate the fathers rights. I would also like to change her name.

Much more information is needed before anyone can properly answer your question. Obtaining custody of a child is not the same as buying a car, you can't just file a paper and be done with it. There are numerous factors which must be considered and that can determine the course of action required.
Your local clerk's office MAY be able to give you some guidance though.
