custody

My mom has had my son for 3 yrs and i need a to know what form i need to give her custody,because im suck and cant take care of him

You need to establish a Permanent Legal Guardianship through the Probate Court. As long as your mother agrees, it is an fairly easy to get it filed. However, once she is appointed his legal guardian, she must file an annual accounting.

If you are sick, maybe he can acquire Social Security benefits. You need to get to the SSA and find out.
