I am 15 years old my dad has custody of me but i want to go live with my mom what papers do i file to do this on my own

Briefly, you cannot. You might find an attorney who could represent you ad litem in the prior custody case. I don't know if it would work though. Also a guardianship might be set up to handle this person/property issue. Either way you are going to need an attorney. You can call one and he probably would give you a short (hopefully) free consultation. Then decide.

The best thing to do is have your mom petition for modification of custody. If your dad contests, you can testify to the judge that you want to live with your mom.
