Easiest way is to go to your local courthouse and pick up the forms from the Clerk's Office. The clerk will tell you what the fees are for this type of filing and the procees to go through

Send the forms your wife has to sign to her and she can then mail them back, as required and notarized.

Once you get the forms back take the whole packet to the clerk's office, file it and get a hearing date.

Or you can hire an attorney and it won't cost you and arm and a leg.
