What form or petition do I need to file an appeal to a judge's decision because my attorney failed to represent me? thank you for your help.

You can't appeal mistakes caused by your lawyer in a family law case, only mistakes made by the judge. If you wish to appeal the judge's decision, you need to look in the Florida Rules of Appellate Procedure. Appeals are very complicated and there are a number of forms that must be filed in the family court and in the appellate court. If you make a mistake with any of the forms, the appellate court can be unforgiving. There are no "downloadable forms" for the appellate court like there are for family court. The appellate court is made for lawyers, not lay people, and it is extremely difficult to file an appeal without a lawyer.
