How do I divorce my wife who lives out of state without knowing her address?

If you do not know her address, you will have to serve her by publication in the newspaper designated by the court in your jurisdiction. During the action, you will have to file an Affidavit of Diligent Search detailing all the steps you took in order to try and locate your wife. If the court is not satisfied that you actually tried to locate your wife, the divorce will not be granted.
