domestic violence/custody of children

I was in a 13 year relationship with this man from which we had two kids. We never married but lived together for most of our relationship. During our relationship we had domestic disputes and also domestic violence which is on record in another state, New York. We separated about two years ago. Since then I received numerous threats over the phone which led me to also put it on record here in Florida where I reside since our separation. He then stopped the threats for some months because we blocked his number and I also changed my number. Now he has resumed threatning not only myself but all of my family members including death threats. My question is what can I do? and is there some kind of legal document that I can get through a Court which legally says that the kids are in my custody and can only be with me incase he decides to take them from me?
