Family Name Change

In 1988 my father filed a petition to have he and his family's name changed (wife and 3 children). We have the petition showing all of our names. Apparently the name change was only done on my father. Tthe final judgement just has my dads name listed. My dad assumed the matching case numbers would have covered it. We were all able to get new social security cards and drivers license.

But apparently this is not the case.

What should I do... an adult name change (pricey), re-open and ask for a modification or amendment, if so which one. This only cost $50.00. Our another family name change for me my mom and my sister.

If you suggest the re-opening of the old case, how and where would I describe the problem.

I already have the paperwork for all of the above options.

contact the attorney who filed this action and have him/her straighten it out. Otherwise, you can open the old case file for $50.00 as you state, ask for a rehearing on the matter, state the facts of the rehearing, set a hearing date, have everyone involved served, and appear for the hearing.
