It's a standard form required in divorces with children and other family law poceedings involving children which lists important information about the kids so that the judge can make sure Florida is the right place for the lawsuit. Primarily, the form requires that each party list where the kids have lived and with whom for the last five years. It also requires information such as whether other lawsuits affecting the children exist in Florida or other states.
