What forms do I need?
What needs to be proven, etc to take away a fathers rights while he is incarcerated ?

You need to file a Petition to Terminate Parental Rights. Just a word to the wise, though, most judges won't terminate parental rights on incarceration alone unless the incarceration is going to last for most--if not all--of the time until the child turns 18.
