Legal Question in Family Law in Florida
I want to file to modify a divorce decree, but I also want to file for contempt/enforcement of a divorce decree for non payment of medical and dental bills, can these be filed together or do I have to file them separately? What forms are needed? I live in St Lucie County Florida

You could try to file a Petition for Modification of Final Judgment and a separate Motion for Contempt/Enforcement. You usually will get the Motion for Contempt heard before the petition for modification.
Don't believe you can file a Petition for Modification of Final Judgment - you could have filed an appeal but that option is not longer available to you. You can file Modification of things in the Final Judgement such as child support, visitation, supporty, payments etc. but not Modify the actual Judgment. As to the non payment of dental and medical bills, yes you can file a Motion for Contempt/Enforcement as to these things - you could ask for a general magistrate hearing on these issues which would happen sooner than waiting for a Judge.
