filing errors & omissions and/or malpractice claims agairst attorney
how do i find the insurance companies covering the attorney and who can help me complete and file the claims?

Write the attorney a letter asking for a copy of his "dec" page. By law, he must supply it to you if he carries malpractice insurance (most attorneys don't).
You may need help from a legal malpractice attorney.

I would send the Attorney a letter asking him to provide you with the Insurance Declarations page for his Malpractice insurance. I suspect you will not be able to file a claim against an Attorney's Malpractice Insurance policy without filing a lawsuit with the assistance of a Legal Malpracitce Attorney.
