Legal Question in Family Law in Florida

I need help Finding Guardianship forms... But, I am wanting the guardianship forms for my childrens god mother to have guardianship over my children if anything happens to me or if i get arrested she will be able to keep my children with her, due to my family is not wanting to help

There are no standard statewide forms like there are for divorce, but some of the clerks' offices have forms online. If your clerk's office doesn't have forms online, you may want to try to google them or buy one of the "help yourself" guardianship books at the bookstore.
