Is there a form to establish visitation between unmarried partners with a child? The parents live in different states and the father is providing support for the child. He wants something in writing regarding visitation and custody of the child in the event something happened to the mother.

Yes, if the child lives in Florida, it's a called a petition for to establish paternity. If the child lives in another state, you'd have to check with a lawyer in the other state.
