What form do I need to file a petition to modify child support from being paid through the courts to directly to the custodial parent in FL, Judical 6?

10 years ago father changed jobs and mother agreed to him paying her directly. Now courts came after father for nonpayment. Mother signed affadavit stating that father has been paying her directly. Child Support Enforcement said now we must file a petition with court to have it modified, but cannot tell us what paperwork, forms, etc. we need to submit. Please help. Father out of work, Mother out of work from MVA. Thank you

There's no standard form for what you want to do. You could take the form for a motion to modify support and change it to explain what you want to do.
