Legal Question in Family Law in Florida

I need to get a form for requesting a court appointed counsel in the 6th circuit court of florida for my indigent son whose paternal rights are being terminated against his wishes. he is currently incarcerated in ohio and needs a request to appear telephonically as well

He has to request that at the next hearing in open court. Make sure you go to the hearing as well
