fullcustody of a child

i received a full custody from the father of my child that he wants no part of him and released full custody to me.. he also wants me to change the childs last name from his to mine.. what letter can i submit to the judge to grant me this petition. or what can i write to the judge to grant me this request.. thank you

I am not sure whether you are asking how to get full custody or if that process is already through and you are just asking about the name change. The former requires many documents and is a more involved process. A name change only requires a few legal documents with a petition at the center and a simple court hearing. It's a petition for name change.

You must institute a custody proceeding and obtain a court order. I suggest you retain an attorney to do it the right way. Good luck.
