Legal Question in Family Law in Florida

I live in Palm Beach County, FL and am going through a Request to Modify Order for Child Support. I spoke with an attorney here that said I can go to the local court house and fill out a Form to Subpeona Bank Account Records. I have called the local court house here and was told that they do not have those forms and to contact the FL Bar. I have tried and was unsuccessful in doing so. How should I do? Is there a website that I am overlooking, that I can get this information from?
