Legal Question in Family Law in Florida

Lost my husband and would like to go back to my maiden name..

Me-and my husband got married on December, 1996, In Florida State. I lost him 2 years later, on 1999. I kept his last name because it was easy to go around. Now, I have got a Florida Driver's license without any problems. The Passport agency, Social Security Office and My Mortgage bank are asking to see the legal name change document. Is there anyway i could get around this? Without paying the fee of $300. Since,I am a widow?

All you need is the Marriage Certificate. You do not need any other document to show that your name change was legal, you certainly don't need to do anything to "keep" it.
