What does this mean?

Partial Modification Mediation Agreement

I thought everything had been dicussed and agreed upon in mediation, but when I look at the court website, this is what it says, although it does say the case has been re-closed.

You should look at the document that is linked to the particular court-docketed item. Sometimes, the clerks do not accurately indicate what type of document was filed with their office. Just by what it says "Partial Modification Mediation Agreement" appears that part of the final judgment was modified at mediation per an agreement.
