What are the minimum visitation / phone call rights of a non-custodial parent?

How about non-custodial grandparents?

Visitation rights are now determined under a parenting plan and some counties have model time sharing plans that are routinely adopted. It used to be that phone calls typically were limited to one phone call a day to say goodnight, but now the child is usually given a cell phone.
