Legal Question in Family Law in Florida

When modifying a divorce in the state of Florida for child custody - if agreed that one child lives with the father and one child lives with the mother, does a child support guideline sheet need to be attached to the modification? The child are 12 and 14 they have made their decisions on who is living with which parent.
