modifying a final divorce judgement

If i have a final judgement and would like to go back in front of a judge to change custody and child support, do i file a supplemental form or a modifacation form?

For those issues, unless there were procedural errors and you can go for setting the judgment aside, you need to petition for modification. However, it is not a simple form. It is an whole process and there must be a good reason, a "substantial change in circumstances" for the petition to be granted.
