I file a motion to move myself and child but court says not done right is there a form to be used

Hi:
Yes there are certain items that must be supplied in a motion of this type for the judge to even consider. Unfortunately , I do not even think that there is a form supplied on the internet that you could follow.
If you want to move, the grounds for the motion must be specific, and potentially address anything that might preclude such a move in a separation agreement that may have been done in your matter.
