Who has custody in the first place? If you have custody, and if it came through the court, then you best file a Motion to Modify that arrangement. That aces out the mother for any kind of visits, then? Why?

If there is no formal custody arrangement, then a notarized statement, witnessed by two witnesses might suffice.

I'd recommend seeing an attorney where you live first though.
