My question would be I am single and pregnant with a baby. I am not sure what my rights are if I should give me baby his father's last name or not. Can you give me any advice on this? If I do give my baby my last name will I have to have his father's permission to change his name later? Do I have to give him his father's last name if I do require him to pay me child support? If I don't will they change his last name anyways to his father's?

Your baby has the right to support and the parents can not decide for the child to not accept that support. Therefore, I suggest that you file a Paternity action to legally establish who the father is. Once that is accomplished the issue of child support can be addressed. At this point, if you are unsure who the father is, you should put your last name for the baby's last name and leave the rest blank where it asks for the father's name. This can be changed later when there is a file determination of the father.
