Legal Question in Family Law in Florida

I am trying to obtain my florida drivers license. When I was born in georgia my birth last name was Lover, when I was seven my mother gave me the last name Knight(no legal papers filed). My SS# card is issued in the name of knight, now the FDLA wants legal papers stating a name change from Lover to Knight. The court house gave me papers to fill out, but they only cover changing my name from Knight to Lover. Please advise.

Correct the papers to show you are changing your name from Lover to Knight and then follow their instructions.
