removing name of non-biological father from birth certificate

The name of the father that is on the child�s birth certificate is not the child�s biological father. Is it possible to have this name removed and to leave this space blank? If so, how?

It will be necessary for you and the biological father to sign affidavits indicating that he is not the biological father. You may contact a family law attorney, Children & Family Services, Clerk of Courts, or Dept. of Public Health, Birth and Death Certificates, to obtain forms and instructions for changing a birth certificate.
