Legal Question in Family Law in Florida

I have shared 50/50 custody with my daughter and have been divorced for almost 5 years. My 15 year old daughter is miserable living with her mother and wishes to live with me full time, Is this possible? After every stay and during her stay I am constantly being called with an upset girl who is saying please Dad, is there any thign you can do? Her education and social abilitlies are being jeopardized. We go to a couselor/ therapist weekly. The mother has attended only 2 times. The counselor is in the opinion that is iwould best suit the child to live with her Dad. I am lost in finding out my recourse but the seriousness seems to be escalating.

Petition the court for modification of child custody, support and visitation. Also Petition for Minor Testimony.
