Legal Question in Family Law in Florida

My spouse refuses to sign a 1172 form for me to renew my spousal Id Card, however, I remain on his Deers and he continues to be compensated for me on his disability and retirement check. He left the state of Florida and I am unable to locate him. Can he be legally made to sign the form being I am still his wife?
