Legal Question in Family Law in Florida

Hi! I am in the state of Florida . I am an unmarried mother of a 4 year old. Her father (Who sigend the birth certificate) has left us again and has had nor made any attempts to have contact with his child. Her last name is hyphenated mine-his. I would like to change her name to just my last name. Can I do this without him? He is not on child support and I cant remember the last time we saw him. How long of "no contact" will allow me to not need him to sign.

He signed the birth certificate. In order to change the birth certificate, you will need his consent. Otherwise you could file for termination of parental rights if he has not seen the child.
