Taking a Child on Vacation

I have visitiation established with my husband (we've been separated since 2005). However, he doesn't take advantage of the visitation. He refuses to speak to me and insists that I go through his girlfriend, which I will not do since I am not married to her.

Do I need to get his permission to take our daughter on vacation across state lines?

No, you do not need his permission to take your daughter on vacation anywhere (though you may need his signature to get her a passport, but that doesn't sound relevant to where you intend to go).
