termination of parental rights
My husband has two boys form a former girlfriend. He has primary custody. She wants to give up her parental rights. Something we have been asking for thru the last 7 years. She does not pay child support or contact the kids. How can we do this. We have no address or phone # for her but, she will be in court on 6-14-07. Are there forms we can have her sign and get this done without an att? Please help/

Your husband can file a Petition to Terminate Parental Responsibilty with the circuit court in your county. I have looked and could only find a sample consent form in conjunction with a stepparent adoption. It can be adapted to fit the needs of your situation. Frankly, your husband should retain an attorney who can draft the appropriate pleadings. This is not a matter to be handled pro se (without an attorney).
