Go to the Courthouse to either the Clerk's Office or the Law Library and inquire as to whether or not they have the forms to file for Visitation.

If there is a prior or pending case file your Motion under the Case No. the Clerk will provide you with and then Notice a hrg before the Judge. Those offices have that form too, I believe.
