Visitation being denied by mother

My husband and his former spouse have been going back and forth with visitation issues since the divorce was final in 96. My husband has not had his children visit his home since Nov,2001, and we live in another county than the children. Recently his ex wife has tried to have an injunction put against me, which was dismissed. Now his ex has had an injunction put against him for domestic violence(something that happened in their marriage in 96 where they were both arrested). What legal actions can he take to try and get visitation back again. We have filed on our own a contempt and a modification on visitation, but she has not yet been served.What other measures can we do legally to have her evaluated psychologically and to have her seek counseling? This can not be good for the two boys in the middle.Thanks in advance
