Legal Question in Family Law in Florida

My ex wife lost custody of our children over 5 years ago due to a drug addiction. Since she has made no effort to complete her case plan through the court. She doesn't call the children, doesn't pay her child support, has been held in contempt for nonpayment twice. At some point, can't her rights be taken away? What should my next steps be?
