Legal Question in Medical Leave in Florida

I requested FMLA paperwork for a leave intermittent leave due a chronic medical situation. Instead of giving me the appropriate form the HR personnel want to interview me about my condition and or issues, they refused to give me the forms without this interview, is this legal.

They do have a right to determine whether your request is legitimate.
