In CA, a declaration requires the following language to make a statement a declaration and therefore admissible as evidence to the court:
"I declare under penalty of perjury under the laws of the state of CA that the foregoing is true and correct"
What language is required to make a declaration in Florida?

Florida doesn't allow declarations. There is a similar form, called an affidavit, which uses the same language, but it is only admissible in very, very rare circumstances. In Florida, the witness has to come to court and testify.
