What are the consequences if the defendant in my case provided false information on a notarized document in Small Claims Court? As the Plaintiff, what should I file or send to the Judge/Court?

You must prove it false. If it is blatantly false, it's perjury. But, as the plaintiff, it is your burden to prove to the court what is true or false. Work on proving your case and impeaching the witness rather than having him/her prosecuted for being a criminal liar. Read up on impeachment. Call me if you want an attorney.
