do I have to give them my ss and drivers license numbers? What about the names and addresses of my grown children?

The form requires it. If you don't follow the form, the other side may go the judge and ask for an order requiring you to answer. If that happens, you may have to pay the attorneys' fees of the other side. Sometimes, even worse penalties are imposed.
